Directions for Downloading the files using a Windows Operating System:

To download the forms, right click your mouse on the pictures above.  In the menu, click on " Save Target As...." if you have Internet Explorer.  If you have Netscape, click on "Save Link As....".  Select a location to download the files to, and click "Save".  To retrieve the files, open Adobe Acrobat Reader.  Click the File Menu, and select "Open".  Locate the file, and select it.  Click "Open".
